Requests for Patient Records
- If you would like a copy of your mental health records, please email a request to [email protected]. Please be sure to include your phone number as we will then be in contact with you regarding your request.
- For minors, a personal representative is a parent, guardian, or other person who has authority to make decisions related to healthcare for the minor. The law indicates that parents have the right to access records of their child's treatment, however, parents should understand the difference between mental health records and medical records and the sensitive information contained within them. Mental health professionals have the right to refuse release of these records to parents or any other entity if they believe that doing so would cause harm to the child. Therefore, the therapist will not release the mental health record of the child to the parent when it is not in the child's best interests to do so, unless required by law. The therapist urges the parents to respect this policy as it is in place to protect the well-being of the child as well as the quality of the parent-child relationship. - If you are requesting records for an adult other than yourself, the client will need to fill out a Release of Information prior to us contacting you regarding your request. If the client is unable to complete a Release of Information due to ability or capacity issues, a valid and notarized Power of Attorney* will be required prior to any response to your request. Please keep in mind that we will not be contacting you regarding an adult client without a Release of Information or a Power of Attorney.
- If the record is needed by a certain date, please indicate this in your initial request. Please note that we will attempt to complete your request as quickly as possible, but it may take up to 30 days to process. We appreciate your patience. Please do not call to make your request as it is required to be in electronic form rather than verbal.
The fee for a records request is $30. This covers labor for copying the requested information, supplies for creating any paper copy or electronic media, the labor to prepare an explanation or summary, and postage.
*A requestor would be considered a personal representative under the Privacy Rule only if this individual has the power to make health care decisions for the client.
