Please verify that your clinician of choice is paneled with your insurance. There is a list under each therapist's photo on the Staff page.

FEE SCHEDULE
- We accept cash, checks, debit, credit, and HSA cards.
- Copays vary depending on your insurance, policy, and deductible. Please check with your insurance company to verify your copay prior to your first visit. It is often noted on your insurance card, though this does not take possible deductibles into consideration.
- Cash pay individual session rate, 50 minutes of counseling $150
- Cash pay couple session, 50 minutes of counseling $170
- Letter writing is not covered by insurance $30
- Records request $15
- Missed appointments with no call $100
- Late cancellations $100
- You are responsible for canceling or changing your appointments. This can be done by calling our scheduling line at (801) 541-7049 or logging into your portal. Calls are taken between 9AM and 5PM Monday through Friday. Confidential voice mail is available during off hours. You must call with a minimum of 24-hours advance notice to cancel or move an appointment.
- Bishop pay:All of our therapists accept LDS bishop pay. If your LDS bishop has agreed to pay for some or all of your counseling, please let us know when you make your appointment so we can work out the details of payment prior to our first therapy session. A Billing Release of Information for your bishop is required and will be emailed to you to fill out.

YOUR RIGHT TO A "GOOD FAITH ESTIMATE"
You have the right to receive a 'Good Faith Estimate' explaining how much your mental health care may cost.
Under the law, we will give clients who do not have insurance, or who are not using insurance, a cost estimate of the bill for mental health services.
- You have the right to receive a 'Good Faith Estimate' for the total expected cost of any services.
- Your care provider must give you a 'Good Faith Estimate' in writing for scheduled services within designated timeframes. You can also ask your care provider for a 'Good Faith Estimate' before you schedule a session.
- If you receive a bill that is at least $400 more than your 'Good Faith Estimate', you can dispute the bill.
- Make sure to save a copy of your Good Faith Estimate.
